Our staffs are highly trained to assist in the day to day activities of our patients. Our staffs will arrange coordination of care with the discharge planners in hospitals and rehabilitation centers to facilitate the return of residents to their home. Managed Care at Home professional staffs will work with your physician to provide continuity of care. All services are supervised and coordinated by a Registered Nurse. Depending on the diagnosis, a Physical Therapist may also coordinate care. By creating a personalized plan of care, we ensured that your recovery is speedy, comfortable and complete. And with the direction of your physician, our professional staffs will work closely with you and your family to provide the care and education necessary for optimum health outcome.
	Managed Care at Home is the vision of its Founder, Family Nurse .. Managed Care at Home is the vision of its Founder, Family Nurse Practitioner Bernarde Udo. Bernarde obtained her Masters Degree in Nursing and has been practicing in the Bay area for over 13 years. Bernarde has worked as the Director of Nursing for San Mateo Hospital and the distinguished Classic Residence by Hyatt serving a diverse population with world – class care. She brings with her an emphasis on caring for the individual with a customized plan of care. It is the fundamental belief of all Managed Care at Home employees that clients simply heal better at home and more successful in maintaining their wellness long-term. As the Administrator of Managed Care at Home, Bernarde will coordinate all care received and visit client personally to develop a comprehensive healing plan.
